Care Coordinator-Public Health Nurse/Registered Nurse/Social Worker

Kinds of Work:
Provide case management, care coordination and comprehensive assessments to Goodhue County residents.
Examples of Work:
Complete MNChoices or South Country Health Alliance comprehensive assessments in home and community settings.
Assess and evaluate client's health and living situations to determine level of care and services needed.
Collaborate with community providers to meet client needs
Provides input in program planning, the development of agency goals and the evaluation of the agency's programs & policies.
Maintains case file documentation.
Work as part of the Waiver and South Country Team.
Knowledge, Skills & Abilities Required:
Ability to establish & maintain effective relationships with clients, staff members, other agency representatives & the general public.
Ability to manage time productively.
Ability to communicate effectively, both orally and in writing.
Ability to make independent decisions and assess client needs & to develop and carry out care plans.
Minimum Requirements:
Bachelor's degree in nursing and certification as a public health nurse with current licensure as a registered nurse in Minnesota or current licensure as a registered nurse or a Bachelor's degree in social work, psychology, sociology or other closely related human services field. Must possess a valid driver's license.
Preferred Qualifications:
One year of nursing or social work experience in home care or long-term care or experience in waiver case management.
The pay rate, depending on qualifications: PHN is $28.12/hour, for RN is $26.81/hour, and for Social Worker/Care Coordinator is $27.84/hour. Full time 40 hours/week.
